Participant and carer/informant surveys

First (baseline) surveys

Thank you for taking the time to complete these surveys. You will have received instructions on which surveys to complete. All the information you provide is kept strictly confidential and secure, only used for research purposes, and is so important to help us achieve our goal: a simple blood test to help improve diagnosis and care for patients and families in the future.

The surveys can be done all at one time, or, you can choose to complete a portion, and come back later to complete the rest.

If you are not sure about any questions, that’s ok, just answer as best as you can, and then later please let us know if we can clarify anything. For any questions, or any feedback, we’d love to hear from you. You can contact us here, via email (contact@themindstudy.org), or phone (03 9342 8750).


Survey 1 – for the Participant to complete

  • Should take approximately 10 minutes
  • These questions help us better understand your quality of life and any symptoms you might have at the moment
  • You can ask someone for help if with understanding any questions, if needed, but please answer all these questions by yourself, as best as you can

Survey 1.5 – for the Participant and/or Carer/Informant to complete

  • Should take approximately 5-10 minutes
  • These additional questions help us better understand your quality of life and any symptoms you might have at the moment
  • You can ask someone for help if with understanding any questions, if needed, but please answer all these questions by yourself, as best as you can
  • Note: a carer/informant can complete this on the participant’s behalf and from the participant’s perspective, if preferred, or in case the participant themselves is unable to 

Survey 2 – for the Participant and/or Carer/Informant to complete

  • Should take approximately 20-30 minutes
  • These questions help us get some more basic details and history, understand what assessments and tests you’ve had, and how much you’d value a blood test that could help accurately and quickly diagnose symptoms and illnesses of the brain and mind
  • You can ask for help from a family member/carer/friend, if needed, with answering any questions in Survey 2
  • Note: a carer/informant can complete this on the participant’s behalf and from the participant’s perspective, if preferred, or in case the participant themselves is unable to 

Survey 3 – for the Carer/Informant to complete (only where the participant has symptoms)

  • Note: only required when the participants has symptoms (i.e. doesn’t need to be completed when the participant is a control / participant without symptoms)
  • Should take approximately 15-20 minutes
  • These questions help us better understand the patient/participant’s symptoms, and if you are a carer, what you are experiencing at the moment, as the carer
